Use this task to create a new concurrency scheme. The configured concurrency schemes are then specified during plan configuration. Connect Back Office comes with a concurrency scheme specifically configured for Medicare. The name of this concurrency scheme is Medicare Default Scheme. The modifier and supervision rules in this concurrency scheme are based on standards published by CMS. In addition to this concurrency scheme, other concurrency schemes can be configured to facilitate the requirements of other plans. A concurrency scheme must be specified at each plan configuration. A default concurrency scheme can be set to use for all plans, and then overridden with a different concurrency scheme for any plans that do not use the default concurrency scheme.

General Concurrency Scheme Information Tab

From the Concurrency Schemes page, click Create. The Concurrency Scheme: New page opens with the General Concurrency Scheme Information tab displayed.

 
2In the General section, enter the name of the scheme in the Scheme Name field. 
3Enter a description of the concurrency scheme in the Description field. 
4Click Apply.

In the Directing Providers section:

  • Update the value in the Supervision begins after n concurrent cases.
  • Enter the two-character modifier for the CRNA, AA, SRNA, and Resident. 
  • Enter the two-character modifier for the supervising physician in the Supervising fields.
  • Enter the modifiers for the SRNA in the Directing CRNA fields.

The n defaults to 4, but can be updated, if needed.

Each level represents the number of cases at which supervision begins. The number of levels corresponds with the n value.

Up to two modifiers can be entered for each provider type. Leave the fields blank if no modifier applies.

In the Solo Providers section:

  • Enter the two-character modifier for the physician acting solo on cases in the Physician fields.
  • Enter the two-character modifier for the CRNA acting solo on cases in the CRNA fields.

In the Directed Providers section:

  • Enter the two-character modifier for the CRNA being directed by the physician in the CRNA fields.
  • Enter the two-character modifier for the Anesthesia Assistant being directed by the physician in the AA fields.

In the Other Providers section:

  • Enter the two-character modifier for the locum tenens physician in the Locem Tenens fields.

In the Monitored Anesthesia Care section, record the two-character modifiers for the following conditions:

  • Deep complex, complicated, or invasive surgery

  • Patient with history of sever cardio-pulmonary conditions

  • Monitored anesthesia care service

 
10Click Apply.

Excluded Procedure Codes Tab

Click the Excluded Procedure Codes tab to add or to remove the procedures included in the concurrency scheme, but excluded from the concurrency scheme rules that detect concurrency exceptions.

Click on add* in the Code field to display the list of procedures, and then do one of the following:

  • Start typing the digits of the procedure code. As you type, the first procedure matching the digits you typed becomes highlighted.

  • Click the scroll bar to locate the procedure to add.

 
13Click on the procedure from the Code drop down list of procedures.   
14Press [Tab]. The procedure code is added to the list of excluded procedure codes.

To delete a procedure code, select it from the list of excluded procedure codes and click Delete.

Click Apply.

Physical Status Modifiers Tab

Click the Physical Status Modifiers tab to configure the physical status modifiers to appear on the claim for payers who require different modifiers than the standard unit values.

Enter the concurrency modifier for the applicable physical status modifier (P1 through P5) for elective and emergency cases in the:

  • Physician fields
  • CRNA fields
  • Anesthesia Assistant fields
 
18(Optional) Check the Send Physical Status Modifiers when Units are Zero checkbox to send zero physical status modifiers when unit values are zero.

If unchecked, the claims will exclude physical status modifiers when the unit values are zero.

19Click Apply.  
20Click Save to save and close the page.
