Concurrency Schemes are the rules and criteria for identifying concurrency exceptions when charges are released in Connect Back Office. Concurrency exceptions occur when a provider exceeds the maximum number of cases that the physician can medically direct or that a Certified Registered Nurse Anesthetist (CRNA) can be involved in at any point, and whether these procedures overlap without appropriate ancillary staff.
The concurrency scheme factors determine a concurrency exception based on the following:
Number of cases a provider can concurrently manage
Level of supervision required for administering anesthesia by a CRNA, Anesthesia Assistant, Student Registered Nurse Anesthetist (SRNA) or resident
Cumulative time that the anesthesiologist CRNA or resident is actually in attendance of the patient
In addition, the concurrency scheme may include modifiers that further define the responsibility of the provider and the services, supervised or unsupervised, that can or must be performed by that provider. Your concurrency schemes can be defined using the appropriate modifiers for your payers and plans. The CPT manual contains a complete list of these modifiers, including definitions and how to use them. Medicare uses a standard set of modifiers that are not listed in the CPT manual.
