The Practice: New and Practices: [name] pages are comprised of eight tabs:

To open the Affiliated Providers tab for new practice:

From the Home Page:	From the Menus:	Via Shortcut Keys:
Click Practices from the Admin list. Click Create. Click the Affiliated Providers tab.	Open the Admin menu and select Practices. Click Create. Click the Affiliated Providers tab.	From the Practices page, press [Alt] + [C]. Click the Affiliated Providers tab.

To open the Affiliated Providers tab for existing practice:

Click Practices from the Admin list.
Select the practice to be modified.
Click Update*.
Click the Affiliated Providers tab.

*Alternate Navigation: Double-click the practice from the list.

Open the Admin menu and select Practices.
Select the practice to be modified.

From the Practices page, select the practice to be modified.

Field Definitions

Field	Type	Required	Description
Type	Radio Selection	Yes	A radio selection option to toggle the view of providers between Practice providers and Referring providers.
Practice	Radio Selection	Yes	The filter for showing providers from the master list. Providers are affiliated to practices and facilities. You can filter by: All Providers (default) - Includes all providers from the provider master list in the provider list. Use this option to view all providers from the provider master list regardless of their affiliations. Affiliated Providers - Filters the provider list by only those providers who are already affiliated with the practice or facility. Use this option to determine which providers from the provider master list are affiliated with the practice or facility. Unaffiliated Providers - Filters the provider list by only those providers who are not already affiliated with the practice. Use this option to determine which providers from the provider master list are not affiliated with the practice or facility.
Status	Checkbox	No	Indicates the provider is affiliated with the practice. A checkmark in the checkbox indicates the provider is affiliated with the practice.
Provider Name	Display Only (Pulled from created/updated Providers)	Yes	The name of the provider.
Specialty	Display Only (Pulled from created/updated Providers)	No	The specialty of the provider.
NPI	Display Only (Pulled from created/updated Providers)	Yes	The national provider identification number of the provider.
UPIN	Display Only (Pulled from created/updated Providers)	No	The unique provider identification number of the provider.
Can Refer	Display Only (Pulled from created/updated Providers)	Yes	The flag to indicate the provider is a referring physician. All providers are designated as either Yes or No.
Can Provide	Display Only (Pulled from created/updated Providers)	Yes	The flag to indicate the provider is a providing physician at the practice. All providers are designated as either Yes or No.

Select All/Select None		Provides a convenient way to affiliate/disassociate all providers with the practice. The button toggles between values upon selection.
