| 1 | From the Facilities page, select the facility to be removed. | |
| 2 | Click Delete. The Delete Facility window opens. | If the below message is displayed, it means the facility is affiliated (linked) to a practice or provider. The affiliation at the practice and provider levels must be eliminated before the facility can be deleted. |
| 3 | To delete the facility, click Yes. To cancel the request, click No. |
