Use this task to delete a facility that is no longer valid or was created in error.

Step-By-Step Guide

From the Facilitiespage, select the facility to be removed.

Click Delete. The Delete Facility window opens.

Note
If the facility is affiliated (linked) to a practice or provider, the affiliation at the practice and provider levels must be eliminated before the facility can be deleted. The Delete button will be disabled if there is an affiliation.

To delete the facility, click Yes. To cancel the request, click No.
