Purpose

This report is used to review information pertaining to practices.

How to Get Here?

From Portal:	From Back Office:
From the Reports tab, locate the Practice Detail report. Click Edit for the report. Click Run Report.	Open the Manage menu and select Reports. From the Reports page > Modern tab, select the Practice Detail report from the list of reports. Click Run (or press [Alt] + [R]).

Field	Description
Practice	The name of the practice.
Address Line 1	The first line of the street address of the practice.
Address Line 2	The second line of the street address of the practice.
City	The city where the practice is located.
State	The state where the practice is located.
ZIP Code	The postal code where the practice is located.
Remittance Address Line 1	The first line of the remittance address.
Remittance Address Line 2	The second line of the remittance address.
Remittance City	The city of the remittance address.
Remittance State	The state of the remittance address.
Remittance ZIP Code	The postal code of the remittance address.
EIN	The unique identification number (EIN) issued by the IRS. Could be Tax ID or SSN.
Physician's Group NPI	The value placed in Physician's Group NPI on the practice.
PA Group NPI	The value placed in PA Group NPI on the practice.
CRNA Group NPI	The value placed in CRNA Group NPI on the practice.
AA Group NPI	The identification number (National Provider Identification) issued to providers for claims submission.
Rendering	Anesthesia Rendering Provider selection found in the General Practice Information tab.
Active Date	The current Accounting Date Lockout for the practice.
Term Date	The term (inactive) date of the practice.
Inactive	Indicates via Yes or No flag if the practice is currently active.
