From the Facilitiespage, select the facility to be removed.

Click Delete. The Delete Facility window opens.

 

Warningnote
 If the below message is displayed, it means the If the facility is affiliated (linked) to a practice or provider. The , the affiliation at the practice and provider levels must be eliminated before the facility can be deleted. Image RemovedThe Delete button will be disabled if there is an affiliation.
